Open carpal tunnel surgery is done as an outpatient. The surgery is performed through a small one to two inch incision in the palm to expose the transverse ligament, which is just over the carpal tunnel.
Between the median nerve and the overlying transverse carpal ligament, a metal guide is situated. The surgeon cuts on top of the metal guide through the ligament. This allows more breathing room for the median nerve, which allows it to regenerate and improve sensation and motor function.
Once the ligament is cut, the incision is closed with sutures, a splint is applied to the wrist, and the patient is usually allowed to go home as soon as the anesthetic wears off. Within a couple weeks, patients often begin hand therapy to regain lost strength and for effective pain control post-operatively.
The endoscopic carpal tunnel release is also done as an outpatient. It is designed to end up with less pain and scarring than open release and attempts to make for a quicker recovery.
The endoscopic surgery incision is smaller than the open one and similarly a metal guide is inserted over the median nerve. A video camera is inserted which sends images to an OR television. The surgeon can see the inside of the hand and wrist.
Using these images, the surgeon cuts the constricting transverse carpal ligament, relieving the median nerve pressure. The instruments are removed, and the wound is closed. A splint may be applied and hand rehabilitation may be begun as with open surgery to recuperate wrist and hand strength.
A number of studies have looked at endoscopic versus open carpal tunnel release. The endoscopic version appears to provide quicker pain relief, faster recovery, and quicker improvement of functional abilities. The long term results appear the same.
Both surgical methods have shown equal efficacy in relieving symptoms of carpal tunnel syndrome overall. Whether or not the increased cost of endoscopic surgery makes the modest benefits appropriate is unclear.
